Injury and Pregnancy Waiver

I am participating at my own risk and waive all claims of every nature against the Minnesota Minx, the Women's Football Alliance, Coaches, Staff, Medical Personnel, and/or anyone associated with the Team and/or League including  organizers, officials, sponsors, facility and any other participating agencies with respect to any personal loss, illness, bodily injury or death resulting from participating in these activities. I declare that I am not pregnant. If I am pregnant or become pregnant and do not know it, I agree to hold harmless the Minnesota Minx, the Women's Football Alliance, Coaches, Staff, Medical Personnel, and/or anyone associated with the Team and/or League including  organizers, officials, sponsors, facility and any other participating agencies for any damage or injury that may result to me or my unborn child as a result of my participating in any way with the Team.

Photo and Media Release

I grant to Minnesota Minx and its sponsors and licensees the right to the free use of my name, voice, picture, video and/or likeness in any broadcast, telecast, advertising, promotion or other account of this event.
 

Tryout Fee Acknowledgement

I acknowledge there is a $35 fee to participate in the Minnesota Minx Tryouts and that this fee grants me access to any and/or all of the tryouts held for the 2025 season.
